Legacy Home Care uses technology to identify the needs of both the caregiver and the client, in an effort to find the perfect match. If for any reason, you are not satisfied with our service, please call us immediately and we will do everything in our power, to address your concerns, including providing a replacement caregiver.
Our desire is that we provide you with a caregiver who can not only meet your needs, but also, be a caregiver with whom you can form a long-lasting relationship. We want to support you and your needs for as long as services are needed.
Registered Nurse (RN) Supervision
Legacy Home Care assigns a Registered Nurse to clients to perform an initial assessment at the start of service. In conjunction with the client and/or family member, the service plan will be developed and will cover many areas related to the patient’s physical and mental health during the assessment. It is desired that a family member involved with the care be present during this visit.
After the assessment is complete, the RN will create a Service Plan for the assigned in home caregiver to follow on a daily and weekly basis. The Service Plan identifies the type of care the client receives and will be updated on a quarterly basis, or when there is a change in client
The Nurse Supervisor will follow up with a phone call to check in on the status of care and then make quarterly visits to the home to monitor client condition and progress.
